Founder's Membership 

1. Details

The Founder's Membership is an opportunity to join PCB at a discounted rate by making an early commitment. Those who join by May 1, 2025 will receive a highly discounted rate that gets even better when you sign up for more than one year.

2. Sign Up

A "Key Member" is required for each individual or family. This individual initiates and becomes the point person for your membership. 

 

Please have a Key Member take a moment to choose your Founder's Membership terms. This Key Member will then be sent an invoice for payment.

 

Each new member will be required to complete  the Patient Forms before being seen.
